    Factors contributing to household-resilience capacity to farming risks: Case study of clam farming in Thai Binh province, Vietnam

    Despite coastal endowment is a unique opportunity for coastal farmers it may be embedded with some risks. Vietnam was ranked 18th in world risk index in 2015 with the vulnerability index of 50.87% (Garschagen, Hagenlocher et al. 2016). In this context, Vietnamese clam farmers have been experiencing increased difficulties. This research focuses on clam production in Thai Binh province which has the largest clam area in northern and northern central coastal Vietnam. The central question of the research was “which factors contributing to farmer’s resilience to clam farming risks”. Results of the research indicated that there are several factors, which altogether explained 66% of variance of resilience among the households, formed three groups including (1) farmer’s ability in gaining experiences from failures and harnessing new opportunities; (2) farmer’s perception toward clam farming risks and its impacts; (3) farmer’s confidence about financial capacity and incomes from diversifying activities which can be used to invest in clam farming. From these research results, several support strategies have been suggested to enhance this capacity of the clam farmers, in order to minimize the losses while maximizing the benefit when people seek for sustainable livelihood

    During the last five years, Vietnam has been one of the countries most affected by climate change. Severe typhoons, flooding, cold spells, salinity intrusion, and drought have affected agriculture production across the country, from upland to lowland regions. Fortunately for Vietnam, continuous work in developing climate-smart agriculture has been occurring in research organizations and among innovative farmers and entrepreneurs. Application of various CSA practices and technologies to adapt to the impact of climate change in agriculture production have been expanding. However, there is a need to accelerate the scaling process of these practices and technologies in order to ensure growth of agriculture production and food security, increase income of farmers, make farming climate resilient, and contribute to global climate change mitigation. This book aims to provide basic information to researchers, managers, and technicians and extentionists at different levels on what CSA practices and technologies can be up scaled in different locations in Vietnam

    Associations of Underlying Health Conditions With Anxiety and Depression Among Outpatients: Modification Effects of Suspected COVID-19 Symptoms, Health-Related and Preventive Behaviors

    Objectives: We explored the association of underlying health conditions (UHC) with depression and anxiety, and examined the modification effects of suspected COVID-19 symptoms (S-COVID-19-S), health-related behaviors (HB), and preventive behaviors (PB).Methods: A cross-sectional study was conducted on 8,291 outpatients aged 18–85 years, in 18 hospitals and health centers across Vietnam from 14th February to May 31, 2020. We collected the data regarding participant's characteristics, UHC, HB, PB, depression, and anxiety.Results: People with UHC had higher odds of depression (OR = 2.11; p < 0.001) and anxiety (OR = 2.86; p < 0.001) than those without UHC. The odds of depression and anxiety were significantly higher for those with UHC and S-COVID-19-S (p < 0.001); and were significantly lower for those had UHC and interacted with “unchanged/more” physical activity (p < 0.001), or “unchanged/more” drinking (p < 0.001 for only anxiety), or “unchanged/healthier” eating (p < 0.001), and high PB score (p < 0.001), as compared to those without UHC and without S-COVID-19-S, “never/stopped/less” physical activity, drinking, “less healthy” eating, and low PB score, respectively.Conclusion: S-COVID-19-S worsen psychological health in patients with UHC. Physical activity, drinking, healthier eating, and high PB score were protective factors

    Safety and efficacy of fluoxetine on functional outcome after acute stroke (AFFINITY): a randomised, double-blind, placebo-controlled trial

    Background Trials of fluoxetine for recovery after stroke report conflicting results. The Assessment oF FluoxetINe In sTroke recoverY (AFFINITY) trial aimed to show if daily oral fluoxetine for 6 months after stroke improves functional outcome in an ethnically diverse population. Methods AFFINITY was a randomised, parallel-group, double-blind, placebo-controlled trial done in 43 hospital stroke units in Australia (n=29), New Zealand (four), and Vietnam (ten). Eligible patients were adults (aged ≥18 years) with a clinical diagnosis of acute stroke in the previous 2–15 days, brain imaging consistent with ischaemic or haemorrhagic stroke, and a persisting neurological deficit that produced a modified Rankin Scale (mRS) score of 1 or more. Patients were randomly assigned 1:1 via a web-based system using a minimisation algorithm to once daily, oral fluoxetine 20 mg capsules or matching placebo for 6 months. Patients, carers, investigators, and outcome assessors were masked to the treatment allocation. The primary outcome was functional status, measured by the mRS, at 6 months. The primary analysis was an ordinal logistic regression of the mRS at 6 months, adjusted for minimisation variables. Primary and safety analyses were done according to the patient's treatment allocation. The trial is registered with the Australian New Zealand Clinical Trials Registry, ACTRN12611000774921. Findings Between Jan 11, 2013, and June 30, 2019, 1280 patients were recruited in Australia (n=532), New Zealand (n=42), and Vietnam (n=706), of whom 642 were randomly assigned to fluoxetine and 638 were randomly assigned to placebo. Mean duration of trial treatment was 167 days (SD 48·1). At 6 months, mRS data were available in 624 (97%) patients in the fluoxetine group and 632 (99%) in the placebo group. The distribution of mRS categories was similar in the fluoxetine and placebo groups (adjusted common odds ratio 0·94, 95% CI 0·76–1·15; p=0·53). Compared with patients in the placebo group, patients in the fluoxetine group had more falls (20 [3%] vs seven [1%]; p=0·018), bone fractures (19 [3%] vs six [1%]; p=0·014), and epileptic seizures (ten [2%] vs two [<1%]; p=0·038) at 6 months. Interpretation Oral fluoxetine 20 mg daily for 6 months after acute stroke did not improve functional outcome and increased the risk of falls, bone fractures, and epileptic seizures. These results do not support the use of fluoxetine to improve functional outcome after stroke

    Household risk management strategies for coastal aquaculture risks: the case of clam farming in Thaibinh province, Vietnam

    peer reviewedEndowed with 3,260 km of coastal line and 112 estuaries, Vietnam has a high potential for aquaculture development. However, long coastal line is also embedded with natural high risks under climate changes and sea level rise. Vietnam is ranked at the 18th in the 2015 world risk index and the vulnerability index of 50.9%. Relying on coastal resources, aquatic farmers have adopted a number of strategies to cope with aquaculture risks. By using the OECD holistic approach, this research used several tools to identify farmer's performance on clam farming practices and their risk management strategies (RMS) in Thaibinh province (the largest area of the clam production in the north and north central coastal part of Vietnam). RSMs are found of diversification and flexibility among farmers. For production risks, the RSMs are: (1) enlarging clam raising size and (2) actively controlling clam production by experience and technical innovations. For market risks, the RSM is searching for more clam market channels in both input and output market. For financial risks, RSMs are (1) securing family from clam farming loss by diversifying livelihood activities and (2) accessing secure financial sources in term or interest and bond conditions. Although some RSMs had resulted positive impacts but in overall, the clam farming risks have not managed well by those strategies due to the limitation in capacity of households comparing with level of risks. To cope better with different risks in clam sector, besides the adjustment in RSMs of farmers themselves, it is necessary to have the intervention from government (from national to local level) to address the aquaculture risks which the farmers cannot handle by themselves, such as (1) addressing the issue of polluted wastewater to the clam field; and (2) more focusing in supporting farmer in linkages to the both formal financial market and output market. In addition, supports for technical training targeting on improving farmer's skills and knowledge in farming decision making and market information is also of high value to clam farmers in coping with farming risks

    Aquaculture Land-Use Policy: The Case of Clam Farming in Thaibinh Province, Vietnam

    Policy-making and enforcement remains centralized in Vietnam. Policies have been formulated with less scientific and public justification, thus being largely bureaucratic and infeasible, and in many cases, they have created plagues for people at the grass-roots levels. This article focuses on the implementation of policies related to intertidal land-use and supports for clam farming in the Thaibinh province as a case study to explore the impacts of policies on clam farming and farmers. During the period of 2011–2013, provincial policies on intertidal land allocation and technical and financial supports had boosted clam farming development in the province to a surprising extent. Rapid expansion of the clam farming area has created significant consequences for the farming sector, as well as farmer’s lives. However, for the same provincial policies, but with different enforcement, different farming outcomes for clam farmers in the three study communes have resulted. Where farmers had more of a voice and choice in bidding for the intertidal areas they preferred, they faced fewer problems. It is, thus, suggested that a more decentralized policy-making and enforcement are needed, in which more scientific assessment and farmer participation are required to not only make government policy more successful in supporting farmers and achieving their expected outcomes, but also to provide farmers with more room to make their own farming decisions from which farming and marketing risks could be mitigated

    Assessment of household risk management strategies for coastal aquaculture: The case of clam farming in Thaibinh Province, Vietnam

    Clam farmers have experienced different types of risks that have been further exacerbated by the rapid expansion of clam farming areas, increased growing densities, and increased market difficulties in recent years in the Thaibinh Province of Vietnam. Most farmers have been seriously affected by production risk, market risk, and financial risk, while a number of others have met with success in almost all of their clam-raising cycles. This study applied a differentiating comparative analysis method and multiple discriminant analysis method to discuss the differences in risk management strategies between and among clam farming households and the impacts of those differences on their success/failure rates. In general, the tactics are related to increase in farm size, the application of technical innovations, diversifying livelihood activities, and accessing secure financial sources all provided better conditions for clam growth, diminished losses, and led to speedier recovery from shocks. To support farmers in managing risks, several government interventions are needed: (1) better re-zoning of clam farming areas in parallel with an increase in the farm size of each household, (2) promoting sustainable linkages between the farmers and the formal financial market and output market, and (3) investing more funding into research and extension related to sustainable clam farming practices and to the improvement of farmers’ skills in cooperative works and management
